Better Care Fund Plan 2017-2019
The Better Care Fund (BCF) is a programme spanning both the NHS and local government which seeks to join-up health and care services, so that people can manage their own health and wellbeing and live independently in their communities for as long as possible.
The Better Care Fund Planning guidance for 2017-2019 was recently published by NHS England. It requires all Local Authority areas to submit revised BCF plans by 11 September 2017.
South Gloucestershire Health and Wellbeing Board are keen to incorporate your views and experiences into the 2017 - 2019 Better Care Fund Plan.

We are working with our colleagues at South Gloucestershire Council to consult on what to include in the Better Care Fund Plan.
We are keen to receive your input to the draft plan to ensure it reflects your experiences and views. The consultation will run from Monday 31 July to Friday 25 August 2017 and can be accessed via South Gloucestershire Council's website.
